For valuable consideration received, I (the “Model”) hereby grant to [INSERT COMPANY NAME], its representatives, employees, and assigns (collectively referred to as "Company"), the irrevocable and unrestricted right to use and publish photographs, videos, audio recordings, and any other media (collectively referred to as "Media") of me, taken or recorded by the Company, for any lawful purpose, including but not limited to advertising, marketing, promotion, education, illustration, and trade.

I understand and agree:

1. OWNERSHIP: I release all claims to any copyright, proprietary interests, or moral rights I may have in the Media.
2. USE: The Company may use, reproduce, distribute, display, modify, adapt, publish, and create derivative works from the Media, in whole or in part, in any medium and for any purpose, without obtaining further consent from me.
3. PAYMENT: I confirm that I have received payment in the amount of [INSERT PAYABLE AMOUNT] as full compensation for the use of my likeness or any media featuring me.
4. NO LIABILITY: I release and discharge the Company from any claims, actions, demands, suits, liabilities, and expenses arising out of or in connection with the use of the Media.
5. NO CONTROL: I acknowledge that I have no control over the ultimate use or dissemination of the Media.
6. SUBLICENSING AND ASSIGNMENTS: The Company may sublicense, assign, or transfer its rights under this release to third parties, including partners, affiliates, and collaborators, without my consent.
7. CAPACITY: I, the Model, warrant and represent that I am not a minor, and do have the legal age or status in my country to sign this Agreement and I am 18 years of age or older. I understand English and I have read this authorization, release, and Agreement in full, prior to its execution, and I am perfectly familiar with its contents. If I am under the age of 18, my parent or legal guardian has reviewed and agreed to this release form on my behalf.
8. GOVERNING LAW: This release shall be governed by and construed in accordance with the laws of [INSERT STATE/COUNTRY OF APPLICABLE LAWS].
9. ENTIRE AGREEMENT: The Model understands English and the Model acknowledges reading the entire Agreement in full and attentively prior to signing, and the Creator is perfectly familiar with its contents. This Agreement shall be binding upon the Model’s heirs, legal representatives, and assigns.
